ACCESS Model Participation — January 2027
Sentinum Health Medical Group is targeting the January 2027 CMS ACCESS Model cohort — an outcomes-based program where we only get paid when patients get healthier. Advisors will help design the clinical protocols and care model that powers our participation.
Comprehensive SDOH Wraparound Care
We're actively delivering Community Health Integration and Principal Illness Navigation services — connecting patients with licensed clinical social workers and community health workers to address the social determinants that drive clinical outcomes.
Sentinum Health Medical Group
We are becoming a medical provider — obtaining a Type 2 NPI, enrolling as an ACCESS participant, and hiring contracted physicians and NPs. Founding advisors will help shape the clinical leadership team and care delivery model.
Who we're looking for
We're assembling a small founding cohort of clinical advisors — physicians who have seen what broken care management looks like and want to help build something better.
What makes a great advisor
- Experience managing complex chronic conditions — diabetes, hypertension, CKD, heart failure, obesity
- Familiarity with value-based care, ACO operations, or Medicare chronic care programs
- Belief that AI can meaningfully improve clinical outcomes — without replacing clinical judgment
- Interest in shaping care protocols, not just reviewing them
- Willingness to engage actively (estimated 2–4 hours per month)
Ideal backgrounds(not exclusive)
- Primary care physicians with large chronic care panels
- Internists or geriatricians with ACO or CCM experience
- Former ACO Medical Directors
- Health tech founders or clinical innovators
- Physicians who have seen the limits of fee-for-service and want to build toward what's next
What founding advisors receive

Founding Equity Stake
Founding advisors receive an equity stake in Sentinum Health — on terms available only to those who join at the beginning. If we succeed, you succeed. That's the only structure that made sense to us.

Founding Physician Advisor Designation
You'll be named publicly as a Founding Physician Advisor — a designation that reflects your role in building this from the ground up, not advising from a distance.
Direct Clinical Influence
You won't attend dinners and nod along. We need your clinical judgment shaping our care protocols, our ACCESS model strategy, and our approach to complex patients. This is an active role for physicians who want to build, not just advise.
