We connect your beneficiaries with licensed clinical social workers and community health workers who address housing instability, food insecurity, transportation barriers, and behavioral health needs — the upstream factors that drive downstream costs.

Co-Management Fees for Your Physicians

The ACCESS Model provides co-management fees directly to your aligned physicians for the beneficiaries we jointly manage. Your network physicians benefit financially from the partnership.
